All prospective students are encouraged to contact Disability Services in the early stages of their college planning. Pre-admission services include meeting with an Access Specialist to provide information about eligibility for academic support services and accommodations, appropriate documentation of disability, housing considerations, and transition issues.
Verification forms for various disabilities are accessible on this website. If you prefer to type information, please click on the editable version of the appropriate verification form. Then, print it and make sure that the qualified diagnosing professional signs it before submission to Disability Services.
The Admissions Application Process
To attend the Ohio State University and use Disability Services services, first complete an application to the University. You can obtain an application from your high school guidance counselor or by contacting:
Office of Undergraduate Admissions
Student Academic Services Building
281 West Lane Avenue
Columbus, Ohio 43210
The admissions application is also available on-line at www.osu.edu/futurestudents
The admissions application process for students with disabilities is the same as that of other students. Disclosure of your disability is optional and voluntary. You may disclose your disability if you feel that your academic performance was adversely affected due to circumstances pertaining to your disability. If you choose to disclose, provide that information on a separate sheet of paper and attach it to the admissions application.
It is advisable to send documentation directly to Disability Services. Your eligibility for services/accommodations from Disability Services is a separate process and is done independently of the Office of Admissions. After documentation is reviewed, Disability Services will send an email notification to your OSU email account, (e.g. firstname.lastname@example.org), acknowledging receipt of your documentation and your eligibility status. Prospective students that do not yet have an OSU email account will be notified via paper letter sent to their home address.
Contact Disability Services early in the admissions process, particularly if you have questions about what documentation is required for eligibility purposes for certain disabilities.
